We accept referrals at any point in pregnancy.

To seek care with our team, call or text us at 289-404-1990 from 8am to 8pm daily. We will respond quickly to set up a time that is convenient for you to meet.

FOR HEALTH CARE PROVIDERS:

You can refer to Dr. Kate Gerster at Evolve Obstetrics. Please indicate that this is a referral for the Chrysalis Team if that is not evident in your referral note. If your client may be difficult to reach, it is ideal if we are invited to an appointment with you to meet them for a ‘warm-hand-off’. You are welcome to call/text us to see if we are available with short notice or to book a future time at 289-404-1990 from 8am to 8pm daily.
