CLIENTS REQUESTING CARE:

To seek care with our team, please complete the intake form below and indicate you are self-referring or reach out to 289-404-1990 from 8am to 8pm daily.

FOR PROVIDERS:

Please use the provider referral form below to refer your clients/patients. We accept referrals from anyone (ex. outreach workers, social workers, physicians etc.). If your client may be difficult to reach, it is ideal if we are invited to an appointment with you to meet them for a ‘warm-hand-off’. You are welcome to call/text us to see if we are available with short notice or to book a future time at 289-404-1990 from 8am to 8pm daily.
